Whether you are a new patient or a returning patient, we want your appointment experience to be positive and productive. Our goal is your goal. We will work with you to help you reach your activity and lifestyle goals. For us to accomplish that, please make sure to bring the following items to your appointment:

  • Name, address, and phone number of the referring physician or healthcare professional.
  • Any medical documents (office/hospital notes, operative reports, progress reports, lab reports, etc.) associated with your orthopaedic past.
  • If any X-rays, MRI, CT, or other radiologic studies are taken at another facility or hospital, you must request and bring the actual film/discs to your appointment.

 

Also bring:

  • A complete list of all medications you currently take—prescription, over-the-counter, supplements and vitamins.
  • Your insurance provider information so we can verify it when you check-in and complete your registration.
  • Dress in comfortable clothes that allow access to your injured extremity or, you will be required to change into an exam gown/shorts.
  • Patients arriving late to their appointment may need to be rescheduled to avoid inconveniencing other scheduled patients.

Among the many benefits of the Steven Chudik MD Shoulder and Knee Injury Clinic is its central location in Westmont, Illinois, and Western Springs, Illinois. Just minutes from Chicago, the Clinic is conveniently located near I-55 (Stevenson Expressway), I-294 tollway, I-88 tollway, I-355 tollway, Illinois Rts. 83 and 34. The offices also are conveniently located between Adventist LaGrange and Hinsdale Hospitals, Advocate Good Samaritan Hospital, and Elmhurst Hospital. There is plenty of free parking and wheelchairs are available for use upon your arrival. Both locations have X-ray suites, cast rooms, and physical therapy suites.

The Westmont office is one of two Hinsdale Orthopaedic offices with a Siemens high-field, open-bore magnetic resonance imaging (MRI) machine. With a 70 cm opening, the largest of any MRI design, patients experience a less tunnel-like environment and its short bore length lets most patients have their heads outside the machine during the imaging process. Most important, this in-house diagnostic capability permits Dr. Chudik to begin treatment quickly rather than delaying his diagnosis and initiation of treatment for days or weeks waiting for results to be sent from a hospital or other MRI center.

Home to the Sports Performance Institute at Hinsdale Orthopaedics, the Westmont office is where Dr. Chudik’s Sports Medicine Injury Clinics are held on Monday nights from 5 to 7 p.m.

MRI and Film Review

Review and discussion of diagnostic results—MRI, X-ray, or ultrasound—are done with the patient (and legal guardian or representative) during a clinic appointment with Dr. Chudik. Results are not given over the phone or by email. Please call 630-324-0402 to schedule an appointment after your diagnostic image(s) are taken. If the image(s) are taken at another facility or hospital, you must request and bring the actual film/discs to your appointment.

Once you and Dr. Chudik determine surgery is the best solution for your injury, the next step is to get it scheduled. Although some injuries can give a false sense of security because they seem to improve or the pain lessens in time, some injuries do not get better on their own. Delaying or postponing surgery can cause the injury to become worse and harder to treat. Therefore, once you’ve had a chance to check your calendar and call your insurance provider to ensure coverage for the hospitals and surgery center where Dr. Chudik performs surgery, you need to call Dr. Chudik’s surgery scheduler at 630-324-0402 to not only book your surgery date but also a pre-operative clinic visit and a post-surgical visit. You also should read the information in the blue surgery folder you were given. The information there will answer most, if not all of your questions.

During the pre-operative visit, Dr. Chudik’s Physician Assistant will examine you, review your medical history and confirm everything is ready to go for surgery. If requested, please make sure you have completed all cardiac or medical clearances and bring your MRI disc and radiology report to this visit. During your visit, Dr. Chudik’s Physician Assistant also will answer any surgery questions and have you meet briefly with a physical therapist who will fit you for a brace, crutches, or other assistive devices. Your post-operative visit usually is scheduled 10 days to two weeks after surgery so your stitches can be removed and your recovery progress monitored. It is very important to schedule all your appointments—pre-, post-, and surgery at the same time—to ensure clinic times are available at the correct time intervals.
