Getting injured at work can be a traumatic experience for the employee and the employer. The law is intended to protect both parties, but navigating through the experience can be confusing and frustrating. Dr. Steven Chudik and his staff at the Steven Chudik MD Shoulder and Knee Injury Clinic in Westmont, Illinois, are committed to providing quality health care for injured workers and will fight for patients to ensure timely treatment and therapy so they can safely return to work as soon as possible.

Dr. Chudik treats all shoulder, elbow, knee and ankle work-related injuries. But, before you make an appointment with Dr. Chudik to examine, diagnose and treat your injury, you need to complete the following steps.

  1. Notify your employer in writing of your injury. Your employer should provide you with the name of their insurance carrier and/or claims’ adjuster, the insurance policy number and contact information.
  2. Call the claims’ adjuster and obtain a claim number. You will need to provide this number, as well as the insurance carrier’s name, contact information, and the name and contact information of your employer when you call to schedule an appointment.
  3. Once you have the information detailed above, call our office at 630-324-0402 and ask to be scheduled with Dr. Chudik for a work injury. You will be connected to Dr. Chudik’s workers’ compensation coordinator, who will make the arrangements and coordinate the necessary paperwork.

As a pioneer in orthopaedic arthroscopic shoulder, knee and revision surgery, and a US News & World Report Top Doctor in orthopaedics, Dr. Steven Chudik with the Steven Chudik MD Shoulder and Knee Injury Clinic in Westmont, Illinois, prides himself on his ability to provide fair and accurate assessments of work-related injuries. For more than ten years, Dr. Chudik has been sharing his highly respected knowledge and orthopaedic expertise with attorneys and legal professionals on work-related injuries. His expertise also has been called upon for chart reviews, narratives, depositions and independent medical exams, as well as for phone consultations and information when orthopaedic questions arise.

To request Dr. Chudik’s assistance with a case, please call his office at 630-324-0402. Depositions are held in his Westmont office every other Monday—depending upon his surgery schedule. Service fees must be received at least two weeks prior to the meeting date. All deposition materials for review should be sent directly to Dr. Chudik’s Westmont clinic at least two weeks prior to the deposition or deadline. If you have a large amount of materials that need to be reviewed, please allow three to four weeks for Dr Chudik to thoroughly read the information you send.

Referring new clients to Dr. Chudik

To refer a workers’ compensation client to Dr. Chudik, have them call 630-324-0402 and ask to be scheduled with Dr. Chudik for a work-related injury. They will be connected to Dr. Chudik’s workers’ compensation coordinator who will make the arrangements and coordinate the necessary paperwork.

Requesting an IME appointment

As a certified independent medical examiner, Dr. Chudik, is regularly asked to see injured workers and provide his expert medical opinion on their condition and long-term prognosis. If you want to refer a client to Dr. Chudik for an independent medical exam, please call Dr. Chudik’s administrative assistant, at 630-324-0402, or send her an email to contactus@chudikmd.com. She will call your client and arrange the appointment.

For patient medical records, please download and complete our Medical Records Request Form and fax to our office. Phone requests are not accepted. Please allow at least seven business days for the retrieval and return of this information. The medical records fax number is 630-794-8669.
